I hereby apply for membership to the Canadian Society for Transfusion Medicine (CSTM).
If accepted, I will promote the objectives of the Society and will abide by its by-laws.

* attach proof of full-time enrollment (copy of student card or letter from program director)

HOW TO PAY ONLINE: Once you have completed the online form you will receive a notification that you have an unpaid invoice. To pay online click on "view invoice online". This will automatically take you to PayPal where you will be able to process your payment accordingly.

HOW TO PAY BY CHEQUE: If you would like to pay by cheque simply send a cheque to the Canadian Society for Transfusion Medicine at the address below.

You will receive an automated message with an updated invoice. If you have paid by cheque you will be sent an updated invoice once your payment is received and processed.

COMMENT PAYER EN LIGNE : Une fois que vous aurez rempli le formulaire en ligne, vous recevrez une notification vous informant que vous avez une facture impayée. Pour payer en ligne, cliquez sur « view invoice online » (voir la facture en ligne). Vous serez automatiquement redirigé(e) vers PayPal d'où vous pourrez effectuer votre paiement.

COMMENT PAYER PAR CHÈQUE : Si vous souhaitez payer par chèque, vous n'avez qu'à faire parvenir un chèque à la Société canadienne de médecine transfusionnelle à l'adresse ci-dessous.
